$100 per 45-50 minute session

Insurance
Heart Change Ministry, LP does not accept insurance payments. However, many insurance carriers provide out-of-network benefits. Services may be covered in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

• Do I have mental health out-of-network insurance benefits?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

A receipt is provided for your services that you can use to file for out-of-network benefits.

Reduced Fee
Reduced fee services are available on a limited basis.

Payment
Cash, check and Master Cards, Visa Cards, or Discover Cards are accepted for payment. Payment is due at time of service.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session. At time of intake, a credit card number must be provided to cover this fee.
